Please complete one form for each rider.
If you don't have a way to print out the form before arrival, you may complete one at the Stables. You may preview the necessary information below:
1. Read and initial each paragraph
2. Check your skill level
3. You must agree to wear a helmet
4. Print your name, address, phone, etc. Adults, please sign your name. A Child must also sign or print his name.
5. Finally, complete the height, weight, and age information so we can get the right size horse and saddle ready.
6. Bring the completed forms with you to the Stables
